On waivers and sign-up forms, can members add initials or typed text?

No, this is not supported at this time.
Will previously signed waivers carry over, or do members have to sign again with each new membership purchase?

Yes, Fitness GM will not assign a duplicate waiver if one has already been signed.

Please note, you can add additional required waivers - beyond the general waivers - when creating memberships.
